The health reform legislation signed into law in March provides a significant number of direct and indirect federal grant funding opportunities for medical providers, hospitals, home health agencies, nursing facilities, colleges and universities and employers. Under several of the grant programs, state and local government and non-profit entities are "flow-through" entities through which eligible applicants can apply for federal grant funding.

Description: There is established a competitive health care workforce development grant program (referred to in this section as the "program") for the purpose of enabling state partnerships to complete comprehensive planning and to carry out activities leading to coherent and comprehensive health care workforce development strategies at the State and local levels.

Description: Director of the CDC, in collaboration with the Secretary, shall award grants to eligible entities to promote positive health behaviors and outcomes for populations in medically underserved communities through the use of community health workers.
Grants awarded under subsection (a) shall be used to support community health workers -
(a) to educate, guide, and provide outreach in a community setting regarding health problems prevalent in medically underserved communities, particularly racial and ethnic minority populations;
(b) to educate and provide guidance regarding effective strategies to promote positive health behaviors and discourage risky health behaviors;
(c) to educate and provide outreach regarding enrollment in health insurance including the Children's Health Insurance Program under title XXI of the Social Security Act, Medicare under title XVIII of such Act and Medicaid under title XIX of such Act;
(d) to identify, educate, refer, and enroll underserved populations to appropriate healthcare agencies and community-based programs and organizations in order to increase access to quality healthcare services and to eliminate duplicative care; or
(e) to educate, guide, and provide home visitation services regarding maternal health and prenatal care.

a) Rebalance their long-term support system so that individuals have a choice of where they live and receive services.

- Demonstration Project to Evaluate Integrated Care around a Hospitalization

Statutory Reference: Sec. 2704 of PPACA
Description: HHS shall establish a demonstration project under title XIX of the Social Security Act to evaluate the use of bundled payments for the provision of integrated care for a Medicaid beneficiary – (a) with respect to an episode of care that includes a hospitalization; and
(b) for concurrent physicians services provided during a hospitalization.
The demonstration project shall be conducted in accordance with the following:
(a) The demonstration project shall be conducted in up to 8 states, determined by the Secretary based on consideration of the potential to lower costs under the Medicaid program while improving care for Medicaid beneficiaries. A state selected to participate in the demonstration project may target the demonstration project to particular categories of beneficiaries, beneficiaries with particular diagnoses, or particular geographic regions of the state, but the Secretary shall ensure that, as a whole, the demonstration project is, to the greatest extent possible, representative of the demographic and geographic composition of Medicaid beneficiaries nationally.
(b) The demonstration project shall focus on conditions where there is evidence of an opportunity for providers of services and suppliers to improve the quality of care furnished to Medicaid beneficiaries while reducing total expenditures under the state Medicaid programs selected to participate, as determined by the Secretary.
(c) A state selected to participate in the demonstration project shall specify the 1 or more episodes of care the state proposes to address in the project, the services to be included in the bundled payments, and the rationale for the selection of such episodes of care and services. The Secretary may modify the episodes of care as well as the services to be included in the bundled payments prior to or after approving the project. The Secretary may also vary such factors among the different states participating in the demonstration project.
(d) The Secretary shall ensure that payments made under the demonstration project are adjusted for severity of illness and other characteristics of Medicaid beneficiaries within a category or having a diagnosis targeted as part of the demonstration project. States shall ensure that Medicaid beneficiaries are not liable for any additional cost sharing than if their care had not been subject to payment under the demonstration project.
(e) Hospitals participating in the demonstration project shall have or establish robust discharge planning programs to ensure that Medicaid beneficiaries requiring post-acute care are appropriately placed in, or have ready access to, post-acute care settings.
(f) The Secretary and each state selected to participate in the demonstration project shall ensure that the demonstration project does not result in the Medicaid beneficiaries whose care is subject to payment under the demonstration project being provided with less items and services for which medical assistance is provided under the state Medicaid program than the items and services for which medical assistance would have been provided to such beneficiaries under the state Medicaid program in the absence of the demonstration project.
- Demonstration Projects to Address Health Profession Workforce Needs/Extension of Family-to-Family Health Information Centers

Eligible Entities: The term "eligible entity" means a state, an Indian tribe or tribal organization, an institution of higher education, a local workforce investment board established under section 117 of the Workforce Investment Act of 1998, a sponsor of an apprenticeship program registered under the National Apprenticeship Act or a community-based organization. Eligible individuals:
(a) IN GENERAL - The term `eligible individual' means a individual receiving assistance under the state TANF program.
(b) OTHER LOW-INCOME INDIVIDUALS - Such term may include other low-income individuals described by the eligible entity in its application for a grant under this section. Amount: $85 million each of fiscal years 2010 through 2014.

Description: HHS, in consultation with the Secretary of Labor, shall award grants to eligible entities to conduct demonstration projects that are designed to provide eligible individuals with the opportunity to obtain education and training for occupations in the health care field that pay well and are expected to either experience labor shortages or be in high demand.
- State Demonstration Programs to Evaluate Alternatives to Current Medical Tort Litigation

Description: HHS is authorized to award demonstration grants to states for the development, implementation, and evaluation of alternatives to current tort litigation for resolving disputes over injuries allegedly caused by health care providers or health care organizations. In awarding such grants, the Secretary shall ensure the diversity of the alternatives so funded.
(a) REQUIREMENTS- Each State desiring a grant under subsection (a) shall develop an alternative to current tort litigation that--
(i) allows for the resolution of disputes over injuries allegedly caused by health care providers or health care organizations; and
(ii) promotes a reduction of health care errors by encouraging the collection and analysis of patient safety data related to disputes resolved under subparagraph (A) by organizations that engage in efforts to improve patient safety and the quality of health care.
- Mental and Behavioral Health Education and Training Grants
Awarding Agency: HHS
Eligible Entities: institutes of higher education; To be eligible for a grant under this section, an institution shall demonstrate--
(a) participation in the institutions' programs of individuals and groups from different racial, ethnic, cultural, geographic, religious, linguistic, and class backgrounds, and different genders and sexual orientations;
(b) knowledge and understanding of the concerns of the individuals and groups described in subsection (a);
(c) any internship or other field placement program assisted under the grant will prioritize cultural and linguistic competency;
(d) the institution will provide to the Secretary such data, assurances, and information as the Secretary may require; and
(e) with respect to any violation of the agreement between the Secretary and the institution, the institution will pay such liquidated damages as prescribed by the Secretary by regulation.
Amount: For the fiscal years 2010 through 2013, there is authorized to be appropriated to carry out this section--
- $8 million for training in social work;
- $12 million for training in graduate psychology, of which not less than $10 million shall be allocated for doctoral, postdoctoral, and internship level training;
- $10 million for training in professional child and adolescent mental health; and
- $5 million for training in paraprofessional child and adolescent work.

Description: HHS may award grants to eligible institutions of higher education to support the recruitment of students for, and education and clinical experience of the students in--
(a) baccalaureate, master's, and doctoral degree programs of social work, as well as the development of faculty in social work;
(b) accredited master's, doctoral, internship, and post-doctoral residency programs of psychology for the development and implementation of interdisciplinary training of psychology graduate students for providing behavioral and mental health services, including substance abuse prevention and treatment services;
(c) accredited institutions of higher education or accredited professional training programs that are establishing or expanding internships or other field placement programs in child and adolescent mental health in psychiatry, psychology, school psychology, behavioral pediatrics, psychiatric nursing, social work, school social work, substance abuse prevention and treatment, marriage and family therapy, school counseling, or professional counseling; and
(d) State-licensed mental health nonprofit and for-profit organizations to enable such organizations to pay for programs for preservice or in-service training of paraprofessional child and adolescent mental health workers.
- Advanced Nursing Education Grants

(a) have as their objective the education of midwives; and
(b) are accredited by the American College of Nurse-Midwives Accreditation

Description -- In general, HHS may award grants to and enter into contracts with eligible entities to meet the costs of projects that support the enhancement of nurse midwifery programs, including educational programs for registered nurses that meet guidelines set by HHS and have as their objective the education of nurses who will upon completion of their studies in such programs be qualified to effectively provide primary health care, including primary health care in homes and in ambulatory care facilities, long-term care facilities, acute care, and other health care settings.
- Nurse Education, Practice, and Retention Grants

Eligible Entities: the term "eligible entity" includes an accredited school of nursing defined by 42 USC Section 296; a health care facility; or a partnership of such a school and facility.

Description: HHS may award grants to, and enter into contracts with, eligible entities to enhance the nursing workforce by initiating and maintaining nurse retention programs. HHS may award grants to, and enter into contracts with, eligible entities for programs:
(a) to promote career advancement for individuals including licensed practical nurses, licensed vocational nurses, certified nurse assistants, home health aides, diploma degree or associate degree nurses, to become baccalaureate prepared registered nurses or advanced education nurses in order to meet the needs of the registered nurse workforce;
(b) developing and implementing internships and residency programs in collaboration with an accredited school of nursing, as defined by section 801(2), to encourage mentoring and the development of specialties; or
(c) to assist individuals in obtaining education and training required to enter the nursing profession and advance within such profession.
In addition, HHS may award grants to eligible entities to improve the retention of nurses and enhance patient care that is directly related to nursing activities by enhancing collaboration and communication among nurses and other health care professionals, and by promoting nurse involvement in the organizational and clinical decision-making processes of a health care facility. HHS may also award grants to, or enter into contracts with, eligible entities to address other areas determined by HHS that are of high priority to nurse retention.
In making awards of grants under this program, HHS shall give preference to applicants that have not previously received an award under the program prior to enactment of the health care reform legislation.
HHS shall fund the grant beyond the second year of such award contingent on the recipient of such award having demonstrated to the department measurable and substantive improvement in nurse retention or patient care as determined by the Secretary. HHS is required to submit an annual report to Congress on the grants awarded and the contracts entered into under the program. Each such report shall identify the overall number of such grants and contracts and provide an explanation of why each such grant or contract will meet the priority need of the nursing workforce.
- Workforce Diversity Grants

Description: HHS may award grants to and enter into contracts with eligible entities to meet the costs of special projects to increase nursing education opportunities for individuals who are from disadvantaged backgrounds (including racial and ethnic minorities underrepresented among registered nurses) by providing student scholarships or stipends for diploma or associate degree nurses to enter a bridge or degree completion program, student scholarships or stipends for accelerated nursing degree programs, pre-entry preparation, advanced education preparation, and retention activities.
5.) Demonstration Program to Integrate Quality Improvement and Patient Safety Training into Clinical Education of Health Professionals

- Medicaid Global Payment Demonstration Projects

- Pediatric Accountable Care Organization Demonstration Project

- Independence at Home Demonstration Program

Statutory Reference: Sec. 3024 of PPACA
Description: HHS shall conduct a demonstration program to test a payment incentive and service delivery model that utilizes physician and nurse practitioner directed home-based primary care teams designed to reduce expenditures and improve health outcomes in the provision of items and services under this title to applicable beneficiaries (as defined in subsection (d)).
The demonstration program shall test whether a model described in paragraph (1), which is accountable for providing comprehensive, coordinated, continuous, and accessible care to high-need populations at home and coordinating health care across all treatment settings, results in--
(a) reducing preventable hospitalizations;
(b) preventing hospital readmissions;
(c) reducing emergency room visits;
(d) improving health outcomes commensurate with the beneficiaries' stage of chronic illness;
(e) improving the efficiency of care, such as by reducing duplicative diagnostic and laboratory tests;
(f) reducing the cost of health care services covered under this title; and
(g) achieving beneficiary and family caregiver satisfaction.
- Extension of the Rural Community Hospital Demonstration Program

Statutory Reference: Sec. 3123 of PPACA
Description: Notwithstanding subsection (a)(2), during the 1-year extension period, the HHS shall expand the number of states with low population densities determined by the Secretary under such subsection to 20. In determining which states to include in such expansion, the Secretary shall use the same criteria and data that the Secretary used to determine the states under such subsection for purposes of the initial 5-year period.
In the case of a rural community hospital that is participating in the demonstration program under this section as of the last day of the initial 5-year period, the Secretary shall provide for the continued participation of such rural community hospital in the demonstration program during the 1-year extension period unless the rural community hospital makes an election, in such form and manner as the Secretary may specify, to discontinue such participation.
- Medicare hospice concurrent care demonstration program

Description: HHS shall establish a Medicare Hospice Concurrent Care Demonstration Program at participating hospice programs under which Medicare beneficiaries are furnished, during the same period, hospice care and any other items or services covered under title XVIII of the Social Security Act (42 U.S.C. 1395 et seq.) from funds otherwise paid under such title to such hospice programs.
Independent Evaluation and Reports-
(a) INDEPENDENT EVALUATION- The Secretary shall provide for the conduct of an independent evaluation of the demonstration program under this section. Such independent evaluation shall determine whether the demonstration program has improved patient care, quality of life, and cost effectiveness for Medicare beneficiaries participating in the demonstration program.
(b) REPORTS- The Secretary shall submit to Congress a report containing the results of the evaluation conducted under paragraph (1), together with such recommendations as the Secretary determines appropriate.
- Demonstration Project Concerning Individualized Wellness Plan

Description: HHS shall establish a pilot program to test the impact of providing at-risk populations who utilize community health centers funded under this section an individualized wellness plan that is designed to reduce risk factors for preventable conditions as identified by a comprehensive risk-factor assessment. HHS shall enter into agreements with not more than 10 community health centers funded under this section to conduct activities under the pilot program. In general, eligible wellness plans may include one or more of the following as appropriate to the individual's identified risk factors: (a) Nutritional counseling
(b) A physical activity plan
(c) Alcohol and smoking cessation counseling and services
(d) Stress management
(e) Dietary supplements that have health claims approved by the Secretary
(f) Compliance assistance provided by a community health center employee
In addition, eligible wellness plans must include the following risk factors:
(a) weight;
(b) tobacco and alcohol use;
(c) exercise rates;
(d) nutritional status; and
(e) blood pressure. Grant awardees must measure program results with respect to risk factors by comparing wellness plan participants and a control group.
- Graduate Nurse Education Demonstration
Awarding Agency: HHS
Eligible Entities: The term "eligible hospital" means a hospital (as defined in subsection (e) of section 1861 of the Social Security Act (42 U.S.C. 1395x)) or a critical access hospital (as defined in subsection (mm)(1) of such section) that has a written agreement in place with --
(A) 1 or more applicable schools of nursing; and
(B) 2 or more applicable non-hospital community-based care settings.
Amount: There is hereby appropriated to the Secretary, out of any funds in the Treasury not otherwise appropriated, $50,000,000 for each of fiscal years 2012 through 2015 to carry out this section, including the design, implementation, monitoring, and evaluation of the demonstration.

Description: HHS, acting through the Director of the CDC, shall award competitive grants to state and local governmental agencies and community-based organizations for the implementation, evaluation, and dissemination of evidence-based community preventive health activities in order to reduce chronic disease rates, prevent the development of secondary conditions, address health disparities, and develop a stronger evidence-base of effective prevention programming.
- Medicaid Emergency Psychiatric Demonstration Project

Eligible Entities: Institute for mental illness that is not publicly owned or operated Amount: Funds appropriated for the demonstration shall remain available for obligation through December 31, 2015. The aggregate amount of payments made to eligible states shall not exceed $75 million

Statutory Reference: Sec. 2707 of PPACA
Description: HHS shall establish a competitive demonstration project grant program under which an eligible state shall provide payment under the state Medicaid plan to an institution for mental diseases that is not publicly owned or operated and that is subject to the requirements of section 1867 of the Social Security Act (42 U.S.C. 1395dd) for the provision of medical assistance available under such plan to individuals who –
(a) have attained age 21, but have not attained age 65;
(b) are eligible for medical assistance under such plan; and
(c) require such medical assistance to stabilize an emergency medical condition
A state shall specify in its application the precise mechanism by which it will ensure that institutions participating in the demonstration will determine whether or not such individuals have been stabilized. This mechanism shall commence before the third day of the inpatient stay. States participating in the demonstration project may manage the provision of services for the stabilization of medical emergency conditions through utilization review, authorization, or management practices, or the application of medical necessity and appropriateness criteria applicable to behavioral health. In selecting state applications for the demonstration project, HHS shall seek to achieve an appropriate national balance in the geographic distribution of such projects. The demonstration project selected by HHS shall be conducted for a period of 3 consecutive years.
- Alternative Dental Health Care Providers Demonstration Project
Awarding Agency: HHS
Eligible Entities: Eligible Entities- To be eligible to receive a grant, an entity shall be within a program accredited by the Commission on Dental Accreditation or within a dental education program in an accredited institution and must be:
(a) an institution of higher education, including a community college;
(b) a public-private partnership;
(c) a federally qualified health center;
(e) an Indian Health Service facility or a tribe or tribal organization (as such terms are defined in section 4 of the Indian Self-Determination and Education Assistance Act);
(f) a state or county public health clinic, a health facility operated by an Indian tribe or tribal organization, or urban Indian organization providing dental services; or
(g) a public hospital or health system;

Announcement Deadline: The demonstration projects funded under this section shall begin not later than 2 years after the date of enactment of this section, and shall conclude not later than 7 years after such date of enactment.
Statutory Reference: Sec. 5304 of PPACA
Description: HHS is authorized to award grants to 15 eligible entities to enable such entities to establish a demonstration program to establish training programs to train, or to employ, alternative dental health care providers in order to increase access to dental health care services in rural and other underserved communities.
- Demonstration Project to Provide Access to Affordable Care
Awarding Agency: HHS/ Health Resources and Services Administration (HRSA)
Eligible Entities: To be eligible to participate in the demonstration project, an entity shall be a state-based, nonprofit, public-private partnership that provides access to comprehensive health care services to the uninsured at reduced fees.
Amount: Each state in which a participant selected by the Secretary is located shall receive not more than $2 million to establish and carry out the project for the 3-year demonstration period.

Statutory Reference: Sec. 10504 of PPACA
Description: Not later than 6 months after the date of enactment of this Act, HHS, acting through the HRSA, shall establish a 3-year demonstration project in up to 10 states to provide access to comprehensive health care services to the uninsured at reduced fees. HHS shall evaluate the feasibility of expanding the project to additional states.
